Effective immediately, our office will no longer email any documents containing PHI (Protected Health Information) such as school physical forms, excusal notes, etc. This is in efforts to keep your child’s health information protected.

If you’re not already signed up for our patient portal, you’re definitely missing out! The portal allows for many things such as:

* Communicate with our office
* Request appointments online
* Access most of your child’s health information securely
* Print your child’s yearly school form
* & more

In order to sign up for the portal all you need to do is contact our office with your email address and we will email you an invitation and even walk you through how to navigate the portal, if needed.

More to know:

When you are in for your child’s yearly physical a school form will automatically be available to you on the portal by the end of your appointment. Therefore, if you do not want the printed copy please let the nurse know at the beginning of the visit so one isn’t printed automatically for you.

Keep in mind, if your child is not up to date on their physical the only school physical form available on your portal will be the most recent physical your child has had. As a reminder, all children should have a physical every year.
